EyeMed, Lincoln Financial, Lincoln VisionConnect, Devoted, Cigna, Aetna, Medicare Advantage Plan, Access, Insight, AARP, Anthem, Humana EyeMed, Sight Protect, United Health Care/Spectera, Traditional Medicare, CareSource Market Place, Prime Time & some VSP plans . If you do not see your insurance listed, please call our office.
If you are unable to make your appointment, please notify us within 24 hours. Repeated No call/No show will result in a $30 fee which must be paid at the time of reschedule for your exam. This does not apply to eyewear consultation.

To stream line your visit, we have created these forms for you to fill out before you arrive. You can fill them out & send them back to us at info@loveyourfaceoptical.com or bring them with you to your appointment. Forms are not required to be completed prior to your appointment.
